I have my assistant creating a merchant account report based on companies I've found or that have been recommended to me.
Some of what is important to me, other than what you wrote, are:
a) can I upload the files easily to Quickbooks Pro - sometimes paying a little more upfront (in fees) can save me lots in the long run (book keeping/accounting). So far, I've only found a bank locally and paypal that provides iif files. (vs. cvs files) Don't konw why more companies don't offer that option.
b) does the company have a set up I can use with my cell phone or a phone. Some of what I do is in person and having such a feature, where I can just swip a card through a small "gaget" and give the card back to my clients would be very helpful.

It really depends on where you are located. Some merchants tend to use a third party processing company - similar to Paypal, 2CO, CCAvenue, etc. Worldpay is one option as well.
Assuming you are in the UK, you might check out Barclay's or the electronic payment gateway 2000Charge.
Assuming you are in Australia, there is Eway (www.eway.com.au).
Assuming you are in Canada, there is PSIGate, Paymentech, and Beanstream.
Cardservice will support international merchants with a United States bank account, United States address, and United States phone number. Some merchants have had success with Valis or eTrade in opening a bank account. You will be limited to about $10,000 each month for at least the first six months. The discount rate will be about 4% and the transaction rate is about .35. I would recommend the LinkPoint gateway for international merchants. It is easier to get set up than Authorizenet.com / Verisign's Payflow. Those two gateways can be set up but it usually takes an extra day for security measures.

If it's any help, I'm going through the process of finding one right now. It's difficult to find all the info you need in one place. Merchant account necessities obviously vary from business to business, but here are a couple resources that I think were helpful...
Merchant Account Reviews (it was tough finding a company that actually reviews merchant accounts without being biased towards whoever they're partnering with. the above site seems pretty good though.)
Merchant Council (just some good general info here.)
